Congratulations on your graduation. I think you will be great in your practice in San Diego. I particularly love that you have explained the supplements and the type of sleep and anxiety they suit. People don't know this, even with the SSRIs. I have a large practice in Los Angeles. I am always looking for good psychiatrists. You're going to hate this question, but do you take any insurance? Thanks so much, Karen.
Thank you! I won’t be accepting insurance at this time, but I can provide a “superbill” — a special receipt that can be submitted to insurance for possible reimbursement. When my practice opens, feel free to reach out if you’d like to learn more!
Thank you. I might have some individuals who would especially prefer an integrative psychiatrist. If you are sending out emails for your opening, here is my email. karen@studiocityclinicalassociates.com
Thanks so much, CJ! 30 years as a PA is incredible. I’m excited to get started out here and to share more about the practice soon. Glad to have you following along!
Thanks for your post and congratulations on your great news. I help people sleep too. I’m trained in CBT-I for insomnia. Very helpful. The whole world needs more sleep right now. ☺️
As a lifelong insomniac what works best for me most of the time is a herbal tincture of valerian/hops and a few drops lavender essential oil rubbed on the soles of my feet. It completely relaxes the body. I did not know about lavender capsules so I'll look for them.
Melatonin never did a thing for me. Neither did magnesium but I've never tried glycanite which I actually have on order.
Thank you for this. As a long time insomniac, retired now so although annoying, at least I can nap if I need to during the day, something I try to avoid if possible but sleep deprivation during the day doesn’t seem to make it any easier to sleep during the wee hours. I tend to sleep in 2 hour ‘segments’ with an hour or two or three in between two or three sleeps. My best sleep comes during the early morning hours so I think my body clock is off. I have done the AF CBTI program online and they recommended I sleep between 3 AM and 10 AM. Not my preference.
Anyway, I alternate between taking 500+ a bit of magnesium glycinate in the evening which helps some, and 200 mg of gabapentin which is great at calming my mind and muscles but I must be sensitive to it because the next day I am leggy and unfocused which I hate. So I defer to the magnesium glycinate more often.
I’ve also had restless legs for years but after finding out that the losartan (25 mg) that I was prescibed along with extended release metropolol (25 mg) exacerbated restless legs (the losartan that is), I got my electrophy to change me to lisinopril 5 mg and that greatly decreased my restless legs. I only wish I had found out that losartan can make restless legs worse. Melatonin also exacerbates restless legs so I don’t take that. And of course beta blockers are known to cause sleep problems. My prescription is for such a small amount you would think it wouldn’t matter that much. These two meds were prescribed almost 20 years ago when I had an Afib episode that after much testing was attributed to a highly stressful event. I’ve only had one other one, about 6 years ago, again after a highly stressful event. Heart seems to be healthy but I do have short small runs of SVT, not enough to alarm my electrophysiologist.
So, after this long lead in, thank you for some other possible recommendations. I just ordered the lavender product to try. I do meditation via Headspace and walk my dog daily using a Swedish all terrain rollator (Trionic Veloped) as I have a slowly disintegrating spine according to radiologists. I prefer to believe it’s a seriously regenerating spine and since the surgical prognosis so far only gives me a 33% chance of making it better with the same percentage making it worse or doing nothing at all, I just keep doing my walking and my chair yoga and I’m still mobile with only occasional bouts of pain. Learning annd practicing mind/body techniques for parasympathetic pain has helped a lot, even though my pain comes from a functional issue. The mind is a wonderful thing ann adaptation is the name of the game. I’m 74 and aging in itself requires adaption if you want to be happy in my opinion. That’s after going through those stages of grief at losing parts of yourself due to the aging process but that’s necessary to come out the other side I think.
Will you be able to do telehealth sessions to people who live in other states than California. I’m a native born Californian but I live in New Mexico now. My husband and I may end up in CA at some point if we are lucky to get really old (a number that we keep pushing further away as we age) in order to access better health services hopefully. In the meantime, I’m interested in what you may offer online once you are settled and set up.
Thank you for sharing your story! I’m hoping to offer telehealth across more states soon and will definitely keep everyone posted. Wishing you better sleep and peaceful walks with your pup!
Melatonin is a mixture of 3 forms...M1...2...3, it's the M2 that contributes to sleep onset & mkt in the US as a Rx Rozerem® ramelteon
Rozerem® has been associated decreased testosterone & increased prolactin, may be prudent to caution those too eager to use melatonin.
Most supplements don't contain the stated on the package.
I find more reasons not to recommend melatonin for those challenged with lack of "sleep"...possibly for jet-lag.
Poor "sleep hygiene" is quite often the cause of poor quality sleep for many individuals.
Although I did not find my note & stand correct (kinda late for myself & didn't research it) memory tells me melatonin can alter the results of a PSA test (?).
I tried the calm aid although I felt like sleeping but had to get up 3 times overnight to go to the bathroom. After that I hesitate to try it again. My focus now is on a grounding sheet. Might give that a try.
Would love to hear your thoughts sometime on the ingredients in the supplement “Cortisol Manager” by Integrative Therapeutics. Safe for routine use? Effective? Thanks so much!
Congratulations on moving to san diego. And i'm so glad to hear that you are one of the few psychiatrists that actually look into supplements.
I'm writing a book right now about my mother and taking care of her before she passed. And it's going to be it interesting thing to develop, because i'm a little antipsychiatry, because of my own experience with getting off of zoloft, and I did it, and it took me several years, and i've been off of it for many years now.
And and then taking care of my mother and her not getting diagnosed properly with dementia and Alzheimer's and all the doctors were saying, oh no, it's just depression. Oh no, it's just anxiety. And I said, you want to live with me for a week, it's not going to take you a week to diagnose her properly. If you live with me for a week, it'll take you probably Three or four hours to see that she has dementia and alzheimer's.
But all the doctors were saying, no, no, it's just depression. No, which just anxiety passing my mother off passing my mother off. They didn't want to deal with her.Even the psychiatrist didn't want to diagnose her with alzheimer's and dementia!
This country in the west is all pills pills. Pills and I view most doctors, most of them as slaves to big pharma.
You could be the exception because you did your work on supplements. And I think it's really sad that the educational system in this country doesn't train doctors, not just psychiatrists, doctors, all doctors. They don't train them in anything more than one semester or one class in nutrition.
What happened to let food be thy medicine?
What happened to that?
Sandragregston.substack.com is my substack. I i'm interested in holistic health, on learning more about herbs for my own health. I study astrology, the hermetic Qabalah and the tree of life, and the hermetic tarot. I was in advanced consciousness studies in 2023 in year 1. Then I got laid off from my full time job march 24- and I had to drop the class! I couldn't afford it.
The was from Center for Spiritual Living.
I do both tarot & astrology readings.
I am in riverside county, 80 miles north of San Diego. I go to Sprouts Farmers market for all my supplemental needs. I have many. You're going to like San Diego!
And I have an interview coming up with the county or riverside behavioral health RUHS. I'll have to write about that sometime later!
I'm glad to hear that you are interested in learning about supplements.
Please, do continue your research in how supplements can help and not only that, but changing your diet can help. A lot of people don't realize how diet affects their mental health and in this country, we do have too many stuff in the food that is not real food. I cannot eat MSG. If I do I'll be knocked out.... Meaning I can't concentrate. I can't think straight and you better let me loose from work because I will know need to sleep for 8 hours. The next day, I won't be 100%. MSG is a nuero toxin.
But if you read anything about that in the united states, you're gonna be hard pressed to find the truth!
I think I want to subscribe to you because I'm very interested in My health.
Another great read packed with valuable insights and recommendations.
Thank you for always supporting my writing!

Thank you and welcome to San Diego. I am looking forward to learning about your practice. Enjoy your graduation. CJ, (30 year fp PA in SD) a new fan.

Dr. Jake is (will):only be licenced in Florida and California hence those in other states cannot be his patients.
Loggy, not leggy.
And electrophysiologist not electrophy - couldn’t scroll to fix my mistakes.
Please do a newsletter on ocd and/or trauma
Thank you for the suggestion. I will add it to my list of potential topics!
Wait a minute…are you a Hurricane?
yes! Graduating from UM Psychiatry Residency!
🙌🏻🧡💚 Me too 😄
